ENROLLMENT PROCESS

Ready to Enroll?

We are excited that you would like your student to enroll in the West Des Moines Community Schools Home School Assistance Program. Our online enrollment process makes it easy for you to complete much of the information at home.

Step 1. Gather information.

Any documents can be scanned and uploaded online.

  1. Proof of residency. All new students must provide the proper documentation to establish proof of residency. The name and address on these documents must match the name and address of the parent or legal guardian of the student(s) being registered.

  2. Immunization Records. Iowa law requires a parent to verify proper immunization against diphtheria, tetanus, pertussis, polio, measles, mumps, rubella, hepatitis B and varicella to enroll in school. 

    Learn more about the types of immunizations needed based on your child’s age.
     

  3. Proof of Age. Proof of age is required for all new students to the WDMCS. In Iowa, a child must be five years old on or before September 15 of the year he or she starts school in order to enroll in kindergarten. For students in grades one through 12, proof is also required before your child can start attending school.

  4. Health History. You will be asked to enter general health history information for each student you register. Depending on the age and/or grade of your student, additional information may be requested.
     

  5. Home Language Survey. All new students to WDMCS are required to fill out a Home Language Survey. If it is evident the student has a native language other than English, the child and family are referred to the child’s designated school for registration and assessment for possible ESOL services. This survey is available in 22 languages.
     

  6. Do Not Release Information. You will be asked if photos or videos of your student can be taken. In addition, this allows student information to be listed in school yearbooks and directories.

Step 2. Enroll Online.

Once submitted, we will contact your child’s prior school (if applicable) to request their records.

Step 3. Complete HSAP Online Form.

This form aids our Enrichment teachers and Supervising teachers in planning for the best opportunities for your children.

Step 5. Complete Course of Study Form

On the CPI Form A, #4 Instructional Program Information, states that a Course of Study must be submitted with the CPI on a separate page.

(Lesson plans will be shown to your supervising teacher during your meetings.)

  1. Fill in student's name and birth date.

  2. Fill in name, address, and phone number of person filing the report.

  3. Make sure current proof of immunization is on file with WDMCS.

  4. Turn in your student(s) course of study plan with the CPI.                    (One student per form!) (Lesson plans will be shared with your supervising teacher during your meetings.)

  5. Fill in number of days you are planning for instruction.                                 (Must be at least 148 per academic year.)

  6. This does not need to be filled out if Dual Enrolling or participating in our Home School Assistance Program (HSAP).

  7. Check if selecting any of the optional services listed on the back of the form. This does include participating in our HSAP. SIGN THE FORM!

  8. Special Education: Mark yes or no if currently identified as a child requiring special education or if you consent to evaluation.

  9. Dual Enrollment:

    1. Mark "yes" if your child is participating in a class or sport at one of the WDMCS schools other than Rex Mathes.

    2. Check all that apply to what your child is dual enrolling for.

    3. Fill in grade level.

    4. List subjects, classes, and/or activities (sports) your child will be participating in at one of the WDM schools other than Rex Mathes.   Please be specific. This does not include participation in our HSAP or checking out a Chromebook.

  10. Mark "yes or no" if you are going to participate in our WDM Home School Assistance Program.
